AboutEmpaverse SM 25/100/1000 Tablet is a triple combination of Empagliflozin 25mg, Sitagliptin 100mg, and Metformin 1000mg used to improve blood sugar control in adults with type 2 diabetes mellitus when dual therapy is inadequate.Usage Instructions: Take twice daily with meals, or as directed by your physician. Swallow whole with water; do not crush or chew (extended release). Take consistently at the same time(s) daily with food to minimize GI upset. Individualize dose based on glycemic response and tolerability.Benefits: Provides intensive glycemic control with three complementary mechanisms, significantly lowers fasting and post-meal blood glucose, reduces HbA1c by 2.5-3.5%, promotes weight loss and reduces cardiovascular risk (empagliflozin 25mg), enhances glucose-dependent insulin secretion (sitagliptin), improves insulin sensitivity and reduces hepatic glucose output (metformin), delays or prevents diabetes complications (retinopathy, nephropathy, neuropathy, cardiovascular events).How it Works: Empagliflozin inhibits sodium-glucose cotransporter-2 (SGLT-2) in renal proximal tubules, blocking glucose reabsorption and promoting urinary glucose excretion (insulin-independent). Sitagliptin inhibits dipeptidyl peptidase-4 (DPP-4), prolonging activity of incretin hormones (GLP-1, GIP) which stimulate glucose-dependent insulin release and suppress glucagon secretion. Metformin activates AMP-activated protein kinase (AMPK), inhibiting gluconeogenesis and enhancing peripheral glucose uptake. Triple action addresses insulin resistance, beta-cell dysfunction, and excessive renal glucose reabsorption.Side Effects: Common: genital mycotic infections (more common with 25mg empagliflozin), urinary tract infections, increased urination, nausea, vomiting, diarrhea, abdominal discomfort, metallic taste, nasopharyngitis, headache, upper respiratory infection. Serious: ketoacidosis (even with normal blood sugar), lactic acidosis (very rare), severe dehydration and hypotension, hypersensitivity reactions (angioedema, anaphylaxis, SJS/TEN), acute pancreatitis, severe joint pain, acute kidney injury, vitamin B12 deficiency with long-term use, Fournier's gangrene (rare).Safety Advice: Educate on ketoacidosis recognition (nausea, vomiting, abdominal pain, fatigue, dyspnea) even with normal glucose. Monitor blood glucose, kidney function (eGFR), and volume status regularly. Maintain adequate hydration, especially in elderly or on diuretics. Temporarily discontinue before surgery, prolonged fasting, acute illness, or iodinated contrast procedures. Avoid excessive alcohol intake. Contraindicated in type 1 diabetes, DKA, severe renal impairment (eGFR <30), or end-stage renal disease. Supplement vitamin B12 if deficient. Report severe abdominal pain, persistent nausea/vomiting, signs of infection, or hypersensitivity reactions immediately.Storage: Store below 25°C. Protect from light, heat, and moisture.
