AboutGemisartan BISO 40/5 Tablet is a combination of Telmisartan 40mg and Bisoprolol 5mg used to treat hypertension (high blood pressure) and provide comprehensive cardiovascular protection in patients requiring higher beta-blocker dose.Usage Instructions: Take once daily at the same time, with or without food. Swallow whole with water; do not crush or chew. Continue even if feeling well. Do not stop suddenly (risk of rebound hypertension, angina, or heart failure exacerbation). Take consistently at the same time daily.Benefits: Provides effective blood pressure control with dual mechanism, significantly reduces risk of stroke, heart attack, and cardiovascular death, provides better BP and heart rate control than monotherapy, reduces myocardial oxygen demand and improves exercise tolerance in angina, manages chronic heart failure (bisoprolol), provides renal protection (telmisartan), suitable for patients with hypertension and coronary artery disease or post-MI, improves compliance with single-pill combination.How it Works: Telmisartan selectively blocks angiotensin II type 1 (AT1) receptors, preventing vasoconstriction, aldosterone secretion, and sympathetic activation. Bisoprolol succinate is a cardioselective beta-1 adrenergic receptor blocker that reduces heart rate, myocardial contractility, and cardiac output, and inhibits renin release. Combined action provides synergistic blood pressure reduction, heart rate control, reduced myocardial oxygen consumption, improved coronary perfusion, and comprehensive cardiovascular and renal protection.Side Effects: Common: dizziness, headache, fatigue, bradycardia (<50 bpm), cold hands/feet, constipation, nausea, dyspnea on exertion. Serious: severe hypotension (SBP <90 mmHg), severe bradycardia, 2nd/3rd degree heart block, worsening heart failure, bronchospasm (especially in asthmatics/COPD), severe allergic reactions.Safety Advice: Contraindicated in severe bradycardia, 2nd/3rd degree heart block, decompensated heart failure requiring IV inotropes, hypotension (SBP <90 mmHg), severe asthma/COPD, sick sinus syndrome, or cardiogenic shock. Use caution in diabetes (may mask hypoglycemia-induced tachycardia), peripheral vascular disease, hepatic impairment, hyperthyroidism, or psoriasis. Monitor heart rate, blood pressure, and for signs/symptoms of heart failure. Do not discontinue abruptly (taper metoprolol over 1-2 weeks to avoid rebound ischemia, hypertension, or arrhythmias).
