AboutGemisartan H Tablet is a combination of Telmisartan 40mg and Hydrochlorothiazide 12.5mg used to treat hypertension (high blood pressure) in patients whose BP is not adequately controlled by telmisartan alone.Usage Instructions: Take once daily at the same time, with or without food. Swallow whole with water; do not crush or chew. Continue even if feeling well. Do not stop suddenly without medical advice. Take consistently at the same time daily.Benefits: Effectively lowers high blood pressure with dual mechanism, reduces risk of stroke, heart attack, and cardiovascular events, provides better BP control than monotherapy, reduces fluid retention (hydrochlorothiazide), provides renal protection (telmisartan), provides 24-hour BP control, suitable for patients with diabetes, chronic kidney disease, or moderate cardiovascular risk.How it Works: Telmisartan blocks angiotensin II receptors (ARB), causing vasodilation and reducing aldosterone secretion. Hydrochlorothiazide is a thiazide diuretic that increases renal sodium and water excretion, reducing plasma volume and peripheral vascular resistance. Together they provide synergistic blood pressure reduction through complementary pathways (RAAS inhibition + volume reduction).Side Effects: Common: dizziness, headache, fatigue, hypotension (especially initially), electrolyte imbalances (hypokalemia, hyponatremia), increased urination, constipation, nausea, back pain, upper respiratory infection, hyperuricemia. Serious: severe hypotension, acute kidney injury, severe electrolyte disturbances, angioedema (rare), severe allergic reactions, gout flare, glucose intolerance, photosensitivity.Safety Advice: Contraindicated in pregnancy (ARBs cause fetal toxicity), severe renal impairment (eGFR <30), anuria, or hypersensitivity to sulfonamides (hydrochlorothiazide). Use caution in diabetes (may worsen glycemic control), gout or hyperuricemia, hepatic impairment, or pre-existing electrolyte imbalances. Monitor blood pressure, renal function (serum creatinine, eGFR), and electrolytes (potassium, sodium, magnesium) at baseline and periodically. May increase blood glucose and uric acid levels. Inform doctor of dizziness, syncope, muscle weakness/cramps, irregular heartbeat, decreased urination, or signs of dehydration. Maintain adequate fluid intake. Avoid potassium supplements or salt substitutes containing potassium unless specifically prescribed. Use sunscreen due to photosensitivity risk.Storage: Store below 25°C in a dry place. Protect from light and moisture. Keep in original packaging. Keep out of reach of children. Do not remove from blister until ready to use.
