AboutSitason GM 2/500 Tablet is a triple combination of Sitagliptin 50mg, Glimepiride 2mg, and Metformin 500mg used to improve blood sugar control in adults with type 2 diabetes mellitus when dual therapy is inadequate.Usage Instructions: Take once or twice daily with meals, or as directed by your physician. Swallow whole with water; do not crush or chew. Take consistently at the same time(s) daily with food to minimize GI upset. Individualize dose based on glycemic response and tolerability.Benefits: Provides comprehensive glycemic control with three complementary mechanisms, significantly lowers fasting and post-meal blood glucose, reduces HbA1c by 2-3.5%, enhances glucose-dependent insulin secretion (sitagliptin), stimulates insulin release (glimepiride), improves insulin sensitivity and reduces hepatic glucose output (metformin), delays or prevents diabetes complications (retinopathy, nephropathy, neuropathy).How it Works: Sitagliptin inhibits dipeptidyl peptidase-4 (DPP-4), prolonging activity of incretin hormones (GLP-1, GIP) which stimulate glucose-dependent insulin release from beta cells and suppress glucagon secretion from alpha cells. Glimepiride binds to SUR1 receptor on beta-cell K+ channels, causing depolarization and insulin release. Metformin activates AMPK, inhibiting gluconeogenesis and enhancing peripheral glucose uptake. Triple action addresses insulin resistance, beta-cell dysfunction, and excessive hepatic glucose production.Side Effects: Common: hypoglycemia (especially with missed meals or excessive exercise), gastrointestinal upset (nausea, diarrhea, abdominal pain, metallic taste), headache, dizziness, upper respiratory infection, weight gain (glimepiride), vitamin B12 deficiency with long-term use. Serious: severe/prolonged hypoglycemia, lactic acidosis (very rare), acute pancreatitis, severe allergic reactions (angioedema, SJS), hepatotoxicity, hemolysis in G6PD deficiency.Safety Advice: Educate on hypoglycemia recognition and management (carry fast-acting carbohydrates). Monitor blood glucose and HbA1c regularly. Assess renal function before initiation and periodically (eGFR). Avoid in eGFR <30 mL/min. Limit alcohol intake. Supplement vitamin B12 if deficient. Contraindicated in type 1 diabetes, DKA, severe renal/hepatic impairment, or acute metabolic stress (sepsis, dehydration). Temporarily discontinue before iodinated contrast procedures and major surgery. Report severe abdominal pain, persistent nausea/vomiting, or signs of hypoglycemia.Storage: Store below 25°C.
